Please fill out the Release and Assumption of Risk form and mail it to us after you have registered for your trip.  If you have access to a scanner you can fill out your form, scan it, and then e-mail the PDF file to us at info@cabinfeveradventures.com.  Or, you can fax the document to 1-867-821-3003 (Please call first) If you have any questions about the Release and Assumption of Risk Waiver form then please do not hesitate to contact us with your questions. 

PLEASE NOTE:  You will be required to sign in person in the presence of a representative of Cabin Fever Adventures a second identical copy of the Release and Assumption of Risk Waiver so that we have an original signed copy on file.  By sending us your faxed or scanned copy of the signed waiver you are waiving any right you have to expect a refund of any kind, should you choose to refuse to sign an identical copy of the Release and Assumption of Risk Waiver in our presence immediately prior to departure on your trip.
